Billing and Insurance
All payments and/or copays are due in full at time of service.
We currently accept cash, personal checks, cashier’s checks, debit cards, and major credit cards (Visa, MasterCard, and Discover).
Absolutely! We offer financing through Care Credit and Wells Fargo.
We are an in-network provider with the following insurance companies: Delta Dental PREMIER, Cigna, and Metlife. As a courtesy to our patients we submit to all dental insurance companies, with the exception of Medicaid, State-aid, and HMO policies. However, keep in mind coverage is subject to restrictions and/or policies determined by the patient’s specific insurance company/policy. We encourage all patients to contact their insurance company for further details.
Uninsured patients are welcome! We believe all patients deserve premier dental care regardless of insurance status.
Scheduling and Appointments
Yes! We are always accepting new patients.
To schedule an appointment we need your full legal name, date of birth, contact information, and insurance information if applicable. If you have dental insurance and you are not the subscriber we will require the subscribers information as well. At your first appointment we will also request permission to take a copy of state issued identification and insurance card if applicable.
Yes! We currently offer free consultations for implants, Six Month Smiles, and second opinions.
Chapko and Associates is proud to offer pediatric dentistry in Belvidere, IL. However, any children under the age of 18 must be accompanied by a parent or legal guardian.
